How do I make or change an Appointment?

• To speak to the Receptionist or Nurse during business hours, please dial 256-737-8000, then press 0.

• The operator will take your message

• Then we will call you back during business hours Monday- Thursday. Note: Messages left after 4:00pm Thursday will not be processed until the following Monday.

• New Patients wishing to be seen for a Lung condition need to have their primary physician send a request for consultation

• New Patients wishing to be seen for a Sleep disorder can make her own appointment and a doctor's referral is not needed




How do I get a Prescription Refill?

• To request a Prescription Refill: Please dial 256-737-8000, then press option# 1.

• Leave the information requested (your Name, your Date of birth, Medication requested, list Dr. Warner as your prescriber, and what Pharmacy you use).

• You will not receive a call back but we have received your message. Do not call more than once.

• Your prescription will be sent directly to your pharmacy by the next business day, Monday- Thursday.

Note: Messages left after 4:00pm Thursday will not be processed until the following Monday.
